Unfortunately I do not take Medicare or Medicaid. I am not credentialed with Medicare. Nutrition sessions would be self-pay.

YES I need a referral or 'Order Form' from your Primary Care Doctor. Please have your Primary Care Doctor send a Referral/ 'Order Form' with a nutrition-related diagnosis to my affiliate Nutrition in Motion’s Fax: (877) 743-5351. This allows for easy insurance billing/coding and to work with your health care providers as a team. Examples of diagnoses are: Low Iron (anemia), Constipation, Diarrhea, Diagnosed Eating Disorder, GERD, Irritable Bowel Syndrome, Lactose Intolerance, Malnutrition, BMI: 25+, Underweight BMI <18.5, Abnormal Weight Loss, Abnormal Weight Gain. Exceptions for referrals are BCBS PPO or Mass General.

An initial for 60 minutes will be $200 and follow-ups are 45 minutes at $150.

On average I see clients for six follow-up visits. This allows us to solidify behavior change and achieve our goals in a realistic step-by-step manner that is sustainable.

The initial nutrition session is all about getting to know you, your lifestyle, your activity level, your current diet, history of eating, food preferences, medical history as it pertains to nutrition, relationship with food, cooking ability and overall health and wellness goals. You’ll leave the initial session with some nutrition education and some nutrition intentions/goals. Your life won’t be flipped upside down in one day, instead I take a slow, behavior change approach to create lasting results.
The follow-ups are where we get into more nutrition education detail and can make more specific short-term goals that will enhance your overall long-term health and performance goal.
